If you are mentally impaired:

  • Did you attend exceptional student (E.S.E.) classes?
  • How was your performance?
  • Can you understand writing at an 8th grade level?

If your reading skills are limited:

  • Can you decipher a newspaper?
  • Can you compose and/or follow easy lists, notes or directions?

If you cannot read:

  • Who usually reads to you?
  • How do you complete paperwork for employment, a driver’s license, and/or Social Security?
  • Are you able to perform arithmetic tasks and/or handle money?
  • Explain any on-the-job or job-related education and how long ago you went through it.
  • Explain any armed forces schooling you underwent and how long ago
